New Child Death Review arrangements in Bexley, Lewisham and Greenwich

Bexley became part of new local tri-borough arrangements with Lewisham and Greenwich for child death reviews in Autumn 2020. Following this, the process for reporting child death notifications has now changed. Under the new arrangements, a single central team, based in Lewisham, supports all aspects of the management and administration of the child death review process, though each borough continues to have its own designated doctor for child deaths.

How to notify a death in the new tri-borough CDOP arrangements

All deaths of children usually resident in Bexley, Greenwich or Lewisham boroughs must be notified via the following link:

Anyone can notify a death and you do not need a log in to notify. If you are not sure that a death has been notified, please submit a notification anyway or contact the Child Death Review central team as soon as possible:
